What Is Counterfeit Xanax?

Much like counterfeit currency, counterfeit Xanax resembles the real thing, but it lacks the same value and usability. These pills are illicitly manufactured in clandestine labs to resemble legitimate alprazolam (brand name Xanax). 

The difference is that there are no quality controls in these illegitimate labs. Some of the ingredients may be similar to actual Xanax but the pills may also contain products not normally found in true Xanax. 

Then why would people buy them? The street Xanax market thrives off supply vs. demand issues. Individuals may want more Xanax than they can obtain from doctors so they seek other sources. Street dealers and social media dealers meet this demand by peddling pills as “pharmaceutical grade” when they are actually counterfeit Xanax. 

Common Ingredients in Fake Xanax

What’s actually inside a fake Xanax? It’s a mystery mix. Typical ingredients include other drugs such as dangerous synthetic opioids like fentanyl and designer benzodiazepines like Etizolam and Flualprazolam. 

This mystery can be deadly. Users believe they are taking Xanax, but they may ingest a more potent opioid instead, which can lead to overdose.

In addition to the risk of overdose, counterfeit Xanax also contains dangerous fillers. You can find ingredients such as chalk, battery acid, and floor wax in these fake pills. 

The Rise of “Pressed” Pills

You may also hear of counterfeit Xanax referred to as pressed Xanax. This term is derived from the counterfeiting process. 

Drug traffickers stamp powder into shapes that mimic legitimate pharmaceutical imprints. So, their pressed Xanax has the Xanax bar shape, but what is inside that bar is far different than legit pharmaceutical Xanax.

This process also creates dangerous “hot spots.” With no quality controls, ingredients are randomly stamped into pressed Xanax, so one pill may contain a lethal amount of a substance (a hot spot) while another contains no substance at all.

How to Spot Fake Xanax

Some counterfeit Xanax pills are virtually indistinguishable from the real thing. The only way to be 100% sure that you’re ingesting legitimate Xanax is to take a pharmacy prescription or have the pills lab tested.

Common Fakes by Color and Imprint

Several counterfeit Xanax “brands” have become common in the street market. These can be identified by their colors and imprints.

White BarsThese common generic white bars may be imprinted with “X ANA X.”
Yellow “School Bus”To identify a yellow bus Xanax fake, look for the imprint R039. Additionally, fakes may be the wrong shade of neon yellow. 
Green “Hulks”These green Hulk Xanax fakes are imprinted with S903. They are often pressed with fentanyl or stronger designer benzos like Clonazolam.
Blue “Boeings”Typically stamped with B707, these are currently highly counterfeited with fentanyl.
Red “Devils”Red devil Xanax, imprinted R666, is a completely illicit street product that is notoriously potent and dangerous. (Pfizer does not make red Xanax.)

The Fentanyl Danger

While there are many dangers involved in taking street Xanax, the core risk today is opioid poisoning from fentanyl. If the pressed pills contain fentanyl, they can easily be deadly: Just two milligrams of fentanyl can be lethal.

In 2025, the Drug Enforcement Administration (DEA) seized more than 47 million counterfeit pills that were laced with fentanyl. The DEA reports that six out of 10 seized counterfeit pills that were laced with fentanyl contained a lethal amount of the drug. 

Recognizing Fentanyl Overdose vs. Benzo Overdose

Any overdose poses health risks, but if bystanders don’t recognize a fentanyl overdose, this can be fatal. They may think the person took a sedative and decide to let them “sleep it off,” but the person is actually experiencing a fatal overdose.

Fentanyl Test Strips

Fentanyl test strips are one way to try to detect the presence of fentanyl in counterfeit Xanax. You can scrape a bit of the pill into water and insert the test strip, and the results on the strip will indicate whether the powder contains fentanyl.

However, this test isn’t foolproof. The portion of the pill that was scraped into the water may not contain any fentanyl, but other parts of the pill might. 

Think of it like a chocolate chip cookie. You can scrape away some crumbs that have no chocolate chips in them, but if you eat the rest of the cookie, you’ll definitely encounter the chocolate.

Health Risks and Side Effects

Additional risks of counterfeit Xanax use include: 

Poly-drug withdrawal

Taking street Xanax often means you’re ingesting both benzodiazepines and opioids, so the withdrawal process is even more complicated. 

Poly-drug overdose

Not realizing the fake Xanax already contains opioids, users may combine street Xanax with alcohol, which can cause respiratory distress.

Unknown potency

Etizolam Xanax and other analogs have longer half-lives and higher potencies than alprazolam. The unknown potency of the pills can lead to multi-day blackouts and amnesia.

Street dosing

Designer benzos that are far more potent than prescription alprazolam pose the risk of seizures.

Treatment and Medical Detox

For those who are ready to detox from street Xanax, it is best to do so through a medical detox process. A cold turkey approach at home can be dangerous, due to the seizure risk from benzos and the severe sickness from opioids.

Medical detox from counterfeit Xanax typically involves a two-stage process.

Stabilization

Clinicians medically stabilize the patient, who may have multiple unknown substances in their system.

MAT (medication-assisted treatment)

Clinicians prescribe FDA-approved medications to manage opioid cravings and set up tapering schedules to wean the person off benzodiazepine dependence.

Emergency Response

Naloxone (Narcan) is a medication that can reverse an opioid overdose. If you know someone who uses street pills, it is advisable to carry naloxone so it is available in the case of an overdose.

Note: Narcan will not reverse a pure Xanax overdose, but because fentanyl-laced Xanax is so common, it should always be administered if breathing is compromised.

Look for a chalky texture, rough edges, uneven color, and easy crumbling. However, the only definitive proof is a chemical test strip or lab analysis.

What does using fentanyl-laced Xanax feel like?

If fentanyl-laced, Xanax produces a more intense, euphoric “high” followed by rapid, heavy sedation and respiratory depression. This differs from the straightforward anti-anxiety relief of alprazolam.

Is “Red Devil” Xanax real?

No legitimate pharmaceutical company manufactures a red Xanax bar. These are exclusively illicit, “pressed” street drugs, known for extremely high toxicity.

Can you die from taking one fake Xanax?
